A bullhorn lip lift shortens the philtral distance — the gap between the nose and the upper lip — by removing a precise crescent of skin at the nasal base. The result is more visible upper-lip vermilion, more tooth show, and a more youthful proportion that filler alone cannot produce.
Why filler is not the same operation
Filler adds volume; it does not shorten the philtrum. In patients with long upper lips, repeated filler creates the 'duck' look without ever solving the underlying proportion.
The scar question
The incision sits hidden at the nostril-sill junction. With careful closure and silicone tape for 8 weeks, the scar fades to a fine line that is invisible in conversation for the vast majority of patients.
