Age Spots vs. Melasma: Why the Right Diagnosis Changes Treatment

Cited Research

Patients frequently arrive asking for laser for 'dark spots' that, on examination, turn out to be melasma — a condition where the wrong laser makes things dramatically worse.

Age spots (solar lentigines)

Sharply demarcated tan-brown spots on sun-exposed skin. Respond reliably to IPL, Q-switched laser, or cryotherapy.

Melasma

Diffuse, symmetric facial pigmentation often related to hormones and sun. Aggressive laser worsens it. Treatment is patient: rigorous sun protection, topical agents (tranexamic acid, hydroquinone), and gentle picosecond laser only after stabilization.
