Most moles are benign for life. A small minority are not. Knowing the difference — and knowing when a mole that 'just bothers you' is actually telling you something — is the most important conversation we have in our minor-surgical clinic.
The ABCDE rule, properly applied
- Asymmetry — one half does not mirror the other.
- Border — irregular, scalloped, or poorly defined edges.
- Color — more than one shade, or recent change.
- Diameter — larger than 6 mm (a pencil eraser).
- Evolution — any change in size, shape, color, or symptom (itching, bleeding).
Cosmetic removal — the right way
Even purely cosmetic mole removal should always be sent to pathology. The cost is minimal; the value is enormous. A 2024 study published on PubMed found that approximately 1 in 100 'cosmetic' moles returned a finding that changed the patient's management plan.
Shave, punch, or excision?
Shave excision is fast and produces a flat scar — appropriate for raised benign moles. Punch excision is best for small lesions where full-thickness sampling matters. Full elliptical excision is required for any mole with concerning features or for cleaner cosmetic outcomes on the face.
